STRIVE Comprehensive Community Services (CCS) would like to partner with you in your recovery journey. We offer support and service facilitation to help you identify and reach your goals.

STRIVE is:

•    Recovery Focused – This program is not about getting by. It’s about getting better.
•    Person Centered – You have a voice and a choice. All services on your recovery plan are selected by you to meet your personal goals.
•    Holistic – Mental illness can affect all areas of your life, but that means recovery touches all areas of your life as well. Your plan can include physical health, mental health, social supports, spiritual health, employment goals, life satisfaction, and community involvement. Have a mental illness or substance abuse addiction diagnosed by a qualified professional.
•    Individual – Your plan is about you and tailored to your individual recovery goals.


STRIVE can serve individuals who meet all of the qualifications below:

•    Live in Vernon or Crawford County.
•    Are already on Medicaid or currently eligible/willing to enroll in Medicaid.
•    Have a mental illness or substance abuse addiction diagnosed by a qualified professional.
•    Are determined to need services (this is determined using the MH/AODA Functional Screen).

Enrollment: There are a few steps to enrolling in 
STRIVE.  

•    Complete an application, Referral for CCS Services Form.
•    Meet with a service facilitator. They will complete a “functional screen” to determine eligibility for the program.
•    Start to think about what recovery looks like for YOU. Your service facilitator will help you with this process, but ultimately you will decide what success looks like. What do you want to be different in your life in six months? A year? Five years?  
•    Start to think about who can support you. Recovery involves healing and self-discovery, which can be rewarding to share with others. This can include friends, neighbors, family… whoever is important to you.

Once your application is processed and your functional screen determines you are eligible for services, the fun and work begin. You will have a support and service facilitator to help you develop your recovery team, recovery plan and recovery outcomes.
