Find Your Vaccine History

The Wisconsin Immunization Registry (WIR) is an online database that tracks vaccine records for WI children and adults. To view your Wisconsin vaccination history and see recommended schedules, you will need to enter your name, birthdate and Social Security number.

Access Vaccines You Need

Contact your healthcare provider, pharmacy or health department to receive recommended vaccinations. Most insurances cover the cost of vaccines but the following programs provide free vaccines.  

Vaccines for Children (VFC) 

Children (18 years and younger) may qualify if they meet at least one of the following:

  • Enrolled in or eligible for Medicaid (BadgerCare or BadgerCare Plus)
  • Uninsured
  • American Indian or Alaska Native (as defined by the Indian Health Care Improvement Act)
  • Have insurance that does not cover vaccines or only covers certain vaccines

Vaccines for Adults (VFA) 

Adults (19 years and older) may qualify if they meet at least one of the following:

  • Uninsured
  • Have insurance that does not cover vaccines or only covers certain vaccines

Please note: Free vaccines through the VFC and VFA programs cannot be provided to individuals whose private insurance covers vaccinations at a doctor’s office or pharmacy.

 **to receive through the health department you must be underinsured, uninsured or have Medicaid or Badgercare
